So . . . If I have someone that is always out of the country and is not a US resident. am I able to buy this service for him using his Yahoo account but my credit card? How does Yahoo know that the person using it is not a US resident, can they tell by his IP?

The only hiccup I can think of is the service may be illegal wherever the person is.

Otherwise, I think they just bill based on the number you called. They also bill you if you want a phone number people can call you at.

For the service provider, its all just internet traffic until it hits a telephone system. Then they have to pay the local carrier for the call. The don't care where you sit so much as what telephone networks the call eventually has to hop on.

Yes. It's relatively trivial to determine the country via IP address.
